You ARE covered for:

Top Hospital Cover provides private hospital accommodation in a private hospital together with theatre fees and other associated hospital costs.
or
Top Hospital Cover with a $250 excess provides hospital accommodation in a private hospital together with theatre fees and other associated hospital costs the excess is payable once per member, per membership year. Couples, Single Parent Families and Family covers have a maximum excess of $500 in any membership year.
or
Top Hospital Cover with co-payment provides hospital accommodation in a private hospital together with theatre fees and other associated hospital costs. The co-payment of $100 is payable for overnight hospitalisation to a maximum of $500 per person. Couples, Single Parent Families and Family covers have a maximum co-payment of $1000 in any membership year. The co-payment does not apply to day stays.

Under all three Top Hospital Covers you are covered 100% up to the Medicare Benefits Schedule Fee for inpatient medical treatment and where the doctor participates in our access gap cover your out-of-pocket costs will be reduced or eliminated.

Together with Extras benefits paid per membership year:

• General dental - 60% of fee charged up to $800 per person

• Major dental - 60% of fee charged $800 per person: $1,000 after 3 years & $1,200 after 5 years

• Orthodontic - 60% of fee charged $500 (2nd year) $850 (3rd, 4th & 5th years) $1,000 (6th plus years) per person and a $2,500 lifetime limit

• Laser eye surgery - $300 per person after 3 years and $500 per person after 5 years maximum lifetime benefit

• Optical - 80% up to $300 per person, per membership year

• Physiotherapy - $40 initial consultation, $30 subsequent consultation

• Chiropractic - $40 initial consultatin, $30 subsequent consultation

• Osteopathy - $40 initial consultation, $30 subsequent consultation

• Podiatry - $40 initial consultation, $30 subsequent consultation

• Naturopathy - $35 initial consultation, $25 subsequent consultation

• Acupuncture - $35 initial consultatin, $25 subsequent consultation

• Reflexology - $35 initial consultation, $25 subsequent consultation

• Shiatsu - $35 initial consultation, $25 subsequent consultation

• Homeopathy - $35 initial consultation, $25 subsequent consultation

• Western & Chinese herbalism - $35 initial consultation, $25 subsequent consultation

• Speech/eye & occupational therapy - $35 initial consultation, $25 subsequent consultation

• Dietetics - $35 initial consultation, $25 subsequent consultation

Combined $550 per person or $900 per family for any single therapy.
Overall membership yearly limit $850 per person or $1,700 per family

• Psychology - $70 initial consultation, $50 subsequent up to $550 per person or $800 per family

• Home surgical nursing - $22 up to $400 per person and $800 per family.

• Orthotics - 80% of the cost up to $200 per person.

• Hearing aids - 100% of the cost up to $1,000 per person.

• Audiology - $32 initial consultation, $22 subsequent consultation

• Pharmacy - after deducting the PBS amount, a benefit of up to $50 per script; overall $400 per person or $800 per family.

• Antenatal classes - $12 per session up to $60 per person.

• Membership fees for registered health organisations such as Diabetes Australia - $20 per person.

• Health First - approved programs combined $200 per person, $400 per family for a single program;
overall $300 single, $600 family, membership year.

• Health appliances - 80% of the cost up to $200 per item with a $400 per person or $800 per family.

• Wellbeing pool - tai chi, pilates & yoga.
